  2. Visit ssa.gov and choose 'Apply for Medicare Only'

  3. Fill out all the information that is required. If you need assistance when filling out the application, click the "related information" icon.

  4. To complete the application, click Submit Now.

You may obtain a receipt confirming approval after your application has been completed. For your history, hold the receipt. You will also obtain an application number that you can use to verify your application status. You will receive a letter in the mail with a decision once your application has been reviewed.
